Applicant must provide identities of all persons/entities with authority to direct management or policies, or having five percent ownership interest

Applicant must clear caregiver criminal history screening (8.370.5 NMAC), including disclosure of felony convictions, civil judgments for fraud/embezzlement/misappropriation, and any adverse actions on licenses/permits

Applicant must provide evidence of sufficient resources to operate for six months, including a credit report with a minimum score of 650 for individuals

Submit a letter of intent with boarding home name, owner/licensee, management company (if any), license type requested, proposed manager's name/resume, anticipated number of residents/beds, physical/mailing address, contact information, anticipated payers, and list of services

Submit a program outline including a list/description of services, projected residents, staffing requirements/duties, staffing plans, admission/discharge criteria, and organizational structure diagram

Submit policies and procedures with a crosswalk to regulations

Fees & Costs
$300.00 annually for initial and renewal applications. Fees are non-refundable. Amended license fee: $300.00.
