Accreditation by Joint Commission (JC), Commission on Accreditation of Rehabilitation Facilities (CARF), or Council on Accreditation (COA)

Submission of a letter of intent including: name of AARTC, legal owner, management company (if any), type of license requested, proposed administrator's resume, anticipated number of residential/non-residential clients, intended population and age range, number of residential beds, physical and mailing address, applicant's contact information, anticipated payers, list of services, behavioral health services division approval letter, and accreditation organization approval letter

Patient notification policy and procedure for family members upon admission

Compliance with all provisions of 8.321.13 NMAC

Biennial license renewal; application and fee due 30 days prior to expiration

Zoning and building approval (certificate of occupancy) from appropriate authority (city, county, or municipality)

Fees & Costs
Current fee schedules must be provided by the licensing authority. Fees must be in the form of a certified check, money order, personal, or business check, or other approved electronic payment method, made payable to the state of New Mexico. Fees are non-refundable.
