    Administrator: at least 21 years old, high school diploma, criminal history screening (8.370.5 NMAC), state-approved certification, able to communicate with residents, not work under influence of drugs/alcohol, evidence of education/experience, three notarized letters of reference, comply with employee abuse registry (8.370.8 NMAC)

    Direct Care Staff: at least 16 years old, adequate education/training/experience, comply with employee abuse registry (8.370.8 NMAC), comply with incident reporting (8.370.9 NMAC), valid NM driver's license (if transporting residents), transportation safety training (if transporting residents), proof of insurance (if transporting residents), clean driving record (if transporting residents), criminal history screening (if not covered by 8.370.5 NMAC)

    Training: 16 hours supervised training for new direct care staff, 12 hours annual training including fire safety, first aid, safe food handling, confidentiality, infection control, resident rights, abuse reporting, smoking policy, quality resident care, emergency procedures, medication assistance, ISP implementation

    Capacity: No more residents than licensed capacity. Each building containing resident activities/services/sleeping rooms must be separately licensed

    Building Codes: Comply with state building code and fire codes (14.7 NMAC). Facilities with 16 or fewer residents are "group R", more than 16 are "group I-1", more than 5 non-self-preserving residents are "group I-2"

    Accessibility: Facilities with 4+ residents must provide accessibility for disabilities (main entry, common areas, at least one bedroom/toilet/bathing facility per 8 residents with 36-inch door clearance, 60-inch wheelchair turning radius for toilet/bathing, ramps with 1:12 slope and handrails if >6-inch rise, 5x5 ft landings at doorways, accessible parking)

Fees & Costs

$300.00 annually for base assessment rate. $300.00 for amended license (additional $25.00 per bed if change of capacity). Application and license fees are non-refundable. Evidence of sufficient financial assets to operate for six months, including a credit report with a minimum score of 650, is required.
