    Maintain a license issued by the Department for each group home.

    Comply with all applicable state and federal laws, including non-discrimination.

    Submit a sworn affidavit of a satisfactory compliance history.

    Submit training and staff development plans.

    Submit written operations and personnel policies & procedures manual, and quality assurance plan.

    Submit program plans describing capacity to implement Section 7.2.5.

    Submit written policies on periodic physical examinations.

    Provide evidence of ability to transport residents, including emergency transportation.

    Submit written policies on medication maintenance and storage.

  • 10

    Submit a letter from the Fire Marshal certifying compliance with fire prevention rules.

    Submit an evacuation plan.

  • 12

    Submit a staffing and work schedule.

  • 13

    Submit an operating budget.

  • 14

    Submit a blueprint (or similar plan) of the group home.

  • 15

    Submit a specific plan for safe and confidential storage of residents' records and medication.
