Must be duly licensed under these regulations and state law.

Fees & Costs
Issuance and renewal fees: up to $150 plus $250 for facilities with less than 100 units/beds, or $400 for facilities with 100 or more units/beds. Background examination fee for initial applications: up to $500. Total fee for facilities with 16 or fewer units/beds: $50.
