Operated by a person providing room, board, and personal care and may provide health-related services to three through five elders who are not related to the person providing the service

Staffed by an on-site manager twenty-four hours per day, seven days per week

Must have a written occupancy agreement with each tenant
- 4
Occupancy agreement must describe fees, charges, rates, refund policy, billing and payment procedures
- 5
Elder group home must comply with local housing codes and rules adopted for special classification by the department
- 6
If medications are administered or stored by an an elder group home, or if medication setup is provided, medications must be administered by a registered nurse, licensed practical nurse, advanced registered nurse practitioner, or properly delegated individual; Medications must be stored in locked storage not accessible to unauthorized personnel, labeled, and maintained in compliance with label instructions and state/federal law.

Fees & Costs
For a two-year initial certification, seven hundred fifty dollars; For a two-year recertification, one thousand dollars; For a blueprint plan review, nine hundred dollars; For an optional preliminary plan review, five hundred dollars.
