    Disclosure of ownership, board members, and any felony convictions or violations of child/dependent adult abuse laws

    Program manager to complete an assisted living management class within six months of hire (6 hours of Iowa-specific training)

    Delegating nurse to complete an assisted living manager class or assisted living nursing class within six months of hire (6 hours of Iowa-specific training)

    All personnel in dementia-specific programs to receive a minimum of eight hours of dementia-specific education and training within 30 days of employment

    Annual dementia-specific continuing education (2-8 hours depending on role)

    Written emergency policies and procedures (natural disasters, fire safety); Operating alarm system connected to each exit door in dementia-specific programs; Approval from fire safety bureau for specialized door locking systems; Program structure and facility to meet requirements adopted by building code and fire safety bureaus; Means to control maximum water temperature to prevent scalding; Private dwelling units with single-action, lockable entrance door; Dwelling units to have at least one room with not less than 120 sq ft, other habitable rooms not less than 70 sq ft; Single occupancy dwelling units not less than 240 sq ft (new construction) or 190 sq ft (converted/remodeled); Double occupancy dwelling units not less than 340 sq ft (new construction) or 290 sq ft (converted/remodeled); Each dwelling unit to contain a bathroom (toilet, sink, bathing facilities) for new construction/converted/remodeled; Each tenant to have access to a 24-hour personal emergency response system; Dementia-specific programs to have one or more staff persons awake and on duty 24 hours a day on site and in the proximate area; Non-dementia-specific programs to have one or more staff persons able to respond to call light/emergent needs and in the proximate area 24 hours a day on site.
