A person shall obtain a license from the Department before operating a group home.

Application for Licensure. An applicant for licensure for a group home shall:

Conduct an on-site review to inspect the physical plant and evaluate the proposed group home; and

In addition, for a large group home, a health inspection report;

Conduct an on-site review to inspect the physical plant and evaluate the proposed group home; and

Shall conduct an on-site evaluation of the group home; and
