Fees & Costs

Survey fee for initial and re-licensure inspection. Revised License fee: $50. License fees based on bed capacity: 1-50 beds: $100; 51-100 beds: $200; 101-150 beds: $300; 151-200 beds: $400; >200 beds: $500.
