Functional needs assessment by physician or licensed health care professional within 60 days prior to admission or 5 working days following admission, and annually thereafter.

Service plan developed within 45 days of admission based on assessment.

Cannot admit or retain individuals requiring a level of service not licensed for, extensive/ongoing nursing care, routine physical/chemical restraints, or likely to cause serious harm.

Administrator must complete at least 8 hours per year of continuing education.

Staff orientation within 15 days of hire and annual in-service training on emergency procedures, policies, residents' rights, abuse prevention, etc.
- 6
Staff must have current first-aid and CPR certification.
