Fees & Costs

Initial, renewal, change of ownership, relocation Type A and Type B: $300 fee plus $15 per bed (max $2,250 for 3-year license). Two-year license: $200 plus $10 per bed (max $1,500). Type C: $150 for 3-year license, $100 for 2-year renewal. Alzheimer Certification (3-year): $300. Increase capacity fee: $15 per bed. Late fee: one-half the basic fee if renewal submitted less than 45 days before expiration.
