  No more than two residents per sleeping room

  Shower/bathing facilities not occupied by more than one resident at a time

  Lockable doors on resident sleeping rooms (unless contraindication)

  No more than four residents sharing toilet facilities

  No more than four residents sharing bathing facilities (with Department approval for special needs)

  Direct care staff are qualified nursing, activity, social and therapy staff employed by or under direct supervisory control

  All staff assisting in or responsible for food preparation must attend Department-approved food service training program
