    Administrator certificate valid for three years, requires 30 hours of continuing education for renewal

    Description of physical plant, including number and type of beds

    Architectural design to promote privacy, choice, independence, dignity and a home-like environment

  • 10

    Physical plant to facilitate care for residents with common long-term care problems

  • 11

    Within 36 months after licensure, at least 20% of residents must have nursing home-level of care needs

$1,500 + $15 per bed (Assisted Living Residence/Comprehensive Personal Care Home initial and annual renewal); $1,125 (Assisted Living Program initial and annual renewal); $1,500 biennial inspection fee (Assisted Living Residence/Comprehensive Personal Care Home); $750 biennial inspection fee (Assisted Living Program)
