Initial License Application: $7,789.94 (3-8 Beds), $9,026.44 (9-19 Beds), $10,819.37 (20-49 Beds), $14,281.56 (50-99 Beds), $18,238.36 (100+ Beds); Renewal License Application: $445.09 Base Fee + $127.35 Per Bed Fee
