$25 for voluntary license|$75 for 3-5 resident homes + $25 per resident beyond 3|$150 for 6+ resident homes + $25 per resident beyond 6|$25 for modification not adding capacity|$25 per additional resident for capacity modification|$150 for additional onsite visits due to noncompliance
